Dance Clinic Booking Form

Please complete the form below and the Director will follow up with an email or phone call to finalise dates and venue.

- A separate invoice will be sent once both parties are satisfied with the dates.

- Payments are required in advance in order to secure studios and venues. 

All Dance Clinics are secured by payment on a first come first served basis. We will always do our best to work with your availability, however please note that this is not always possible with studio availability and our peak times. Fees are non-refundable.​​​

Student Waiver Form

I understand that participation in dance classes, dance clinics, rehearsals and performances with The Sussex Dance Company involves physical activity and carries risks of personal injury. I agree that my child/ren will participate voluntarily at our own risk.

I release The Sussex Dance Company, its teachers, and affiliates from any responsibility for injury, loss, or damage sustained during classes, rehearsals, performances, or related activities, except in cases of proven negligence.

I confirm that my child/ren is in good health and that any relevant medical conditions have been disclosed to the instructor in advance. I consent to basic first aid in the event of an injury or medical emergency.

​

I also give permission for photographs or videos taken during classes or events be used for educational or promotional purposes by The Sussex Dance Company.
