Term Registration Form

New students and families are required to complete the mandatory Registration Form below. Once we receive the completed form, an invoice will be sent via email (please check your spam folder).

 

Your child/ren are not considered registered until we receive the Registration Form and Full Term Payment. Your child/ren will be placed on a waiting list until payment has been made. All fees are non-refundable once payment has been made.​​

​All information will be treated as confidential, please make sure to update your contact details via emailing us at info.sussexdancecompany@gmail.com.

Current SDC Families receive £15 off Term Fees if they refer new families. New referred families will receive £10 Discount Welcome Offer. You must let us know at the time of registration in order to receive the Welcome Offer.

Student Consent & Waiver Form

I understand that participation in dance classes, rehearsals and performances with The Sussex Dance Company involves physical activity and carries risks of personal injury. I agree that my child/ren will participate voluntarily at our own risk.

I release The Sussex Dance Company, its teachers, and affiliates from any responsibility for injury, loss, or damage sustained during classes, rehearsals, performances, or related activities, except in cases of proven negligence.

I confirm that my child/ren is in good health and that any relevant medical conditions have been disclosed to the instructor in advance. I consent to basic first aid in the event of an injury or medical emergency.

​

I also give permission for photographs or videos taken during classes or events be used for educational or promotional purposes by The Sussex Dance Company. Please email info.sussexdancecompany@gmail.com if you DO NOT consent for your child's image to be shared online and social media.
