Adult Class Booking Form

We are delighted for you to join our SDC Adult Dance Programme and can't wait to dance with you!

All participants are required to fill in our form once, including the Waiver Form, before attending our sessions. 

Payment can be made online before the class starts in order to secure your place.

Block Bookings will be billed an invoice.

** Please note that fees are non-refundable but can be credited up to 2 weeks**

A £2 processing fee applies to all online transactions.

Block Sessions are valid for 3 months. An invoice will be sent by email to secure your spot., the £12 will be deducted from the invoice.

General Waiver Form

I understand that participation in dance classes, rehearsals and performances with The Sussex Dance Company involves physical activity and carries risks of personal injury.

I release The Sussex Dance Company, its teachers, and affiliates from any responsibility for injury, loss, or damage sustained during classes, rehearsals, performances, or related activities, except in cases of proven negligence.

I confirm that I am in good health and that any relevant medical conditions have been disclosed to the instructor in advance. I consent to basic first aid in the event of an injury or medical emergency. In the event an injury or medical emergency arises during the session, I agree to stop participation immediately and notify the instructor.

I also give permission for photographs or videos taken during classes or events be used for educational or promotional purposes by The Sussex Dance Company. Please email info.sussexdancecompany@gmail.com if you DO NOT consent for your image to be shared online and social media.
