New students and families are required to complete the mandatory Registration Form and Student Waiver Form below. Once we receive the completed form, an invoice will be sent via email (please check your spam folder).

Your child/ren's place is not secured until we receive both Registration Forms AND full payment. Payment can be sent directly via bank transfer, please find payment details at the end of the form.

Student Consent & Waiver Form

I understand that participation in dance classes, rehearsals and performances with The Sussex Dance Company involves physical activity and carries risks of personal injury. I agree that my child/ren will participate voluntarily at our own risk.

I release The Sussex Dance Company, its teachers, and affiliates from any responsibility for injury, loss, or damage sustained during classes, rehearsals, performances, or related activities, except in cases of proven negligence.

I confirm that my child/ren is in good health and that any relevant medical conditions have been disclosed to the instructor in advance. I consent to basic first aid in the event of an injury or medical emergency.

I also give permission for photographs or videos taken during classes or events be used for educational or promotional purposes by The Sussex Dance Company. Please email info.sussexdancecompany@gmail.com if you DO NOT consent for your child's image to be shared online and social media.
